Pre-visit forms are required for new patients, annual hormone renewal visits, and anyone who hasn’t been seen in 12+ months. Forms are sent by email right after booking and can also be completed using the direct link in your Appointment Launchpad.
When Forms Are Required
You only need to complete pre-visit forms in these situations:
New patient appointments (first visit)
Annual Hormone Care Program renewal visits
If it’s been 12+ months since your last visit — even if you’ve been a patient before
If your visit doesn’t fall into one of these categories, you typically won’t need to complete new forms.
Important Before You Begin
Use your full legal name (as it appears on your ID/medical records). This helps your chart match correctly and avoids delays.
Complete your forms at least 48 hours before your appointment to avoid rescheduling.
New Patients: Intake Forms
If this is your first appointment with Precision FemCare, you’ll complete new patient intake forms.
How to access your intake forms:

Annual Renewals or 12+ Months Since Last Visit: Update Forms
If you booked an Annual Hormone Evaluation — or it’s been more than 12 months since your last visit — you’ll complete annual update forms.

Look for the “Start My Update Forms” link or button (takes about 10 minutes)
These updates help keep your chart current, including any new symptoms, medications, diagnoses, or major life changes.
What If I Don’t Complete My Forms in Time?
If your forms aren’t completed at least 48 hours before your visit, your appointment may need to be rescheduled so your chart can be properly prepared in advance.
